ABSTRACT

RESUMO O objetivo deste estudo foi relatar a abordagem interdisciplinar no manejo da macroglossia em um caso de paciente com síndrome de Beckwith-Wiedemann, no período de dez anos. O acompanhamento iniciou pela equipe de Cirurgia Bucomaxilofacial, seguido da Fonoaudiologia, em função de dificuldades alimentares. Após avaliação clínica e instrumental, aos 8 meses de idade, iniciou-se a intervenção fonoaudiológica com foco na disfagia orofaríngea e na terapia miofuncional orofacial. Foi verificado, com 1 ano e 11 meses, ausência de sinais de alteração de deglutição em fase faríngea e melhora na postura de lábios e língua. Aos 3 anos, foram iniciados estímulos para retirada dos hábitos orais e o treino da função mastigatória. O tratamento ortodôntico para correção de mordida aberta anterior e mordida cruzada posterior unilateral iniciou-se aos 6 anos. Aos 7 anos e 5 meses de idade, constatou-se estabilidade do modo respiratório nasal e adequação da postura de repouso de lábios e língua. Aos 9 anos, em função de recidiva das alterações oclusais, optou-se pela redução cirúrgica da língua seguida de terapia miofuncional orofacial, retomada aos 9 anos e 3 meses. O resultado foi a correção da postura da língua na deglutição e a adequação da fala. A associação dos tratamentos, envolvendo Fonoaudiologia, Ortodontia e Cirurgia Bucomaxilofacial foi considerada efetiva no manejo da macroglossia, resultando na adequação e equilíbrio das funções orofaciais.


ABSTRACT This study aims to report the interdisciplinary management of macroglossia in a Beckwith-Wiedemann syndrome patient during ten years. Clinical follow-up started by the Oral and Maxillofacial Surgery team, followed by Speech Therapy due to feeding difficulties. After clinical and instrumental evaluation, at 8 months old, the speech therapy intervention was indicated, focusing on oropharyngeal dysphagia and orofacial myofunctional therapy. At 1 year and 11 months, no signs of swallowing alteration in the pharyngeal phase and improvement in the posture of the lips and tongue were found. At the age of 3, stimulation to remove oral habits and train masticatory function were initiated. Orthodontic treatment to correct anterior open bite and unilateral posterior crossbite started at age 6. At 7 years and 5 months, there was stability in the nasal breathing mode and adequacy of resting posture of lips and tongue. At the age of 9, due to relapse of the occlusal alterations, surgical reduction of the tongue was indicated, followed by orofacial myofunctional therapy, restarted at the age of 9 years and 3 months. The result was the correction of the posture of the tongue during swallowing and speech adequacy. The association of treatments involving Speech Therapy, Orthodontics and Oral and Maxillofacial Surgery was considered effective in the management of the macroglossia. It resulted in the adequacy and equilibrium of orofacial functions.

ABSTRACT

Introdução: A apicotomia é uma técnica, que vem sendo utilizada em dentes com dilaceração radicular acentuada, com o intuito de liberar a porção do dente impactada e, dessa forma, permitir seu reposicionamento na arcada dentária. Este trabalho tem como objetivo apresentar um relato de caso clínico do manejo ortocirúrgico de tracionamento ortodôntico associado à apicotomia em dente incluso com dilaceração radicular acentuada. Relato de caso: O paciente foi encaminhado para o serviço de bucomaxilofacial para exodontia do elemento incluso após insucesso de tracionamento ortocirúrgico, em que se verificou, ao exame radiográfico periapical, dente incluso com dispositivo ortodôntico e dilaceração radicular importante. Diante disso, realizou-se, sob anestesia local, a apicotomia e instalação do dispositivo ortodôntico. O paciente evoluiu bem, e, após 8 meses, o elemento dentário encontrava-se em posição oclusal e em função mastigatória. Considerações finais: Assim, a técnica da apicotomia mostra-se eficaz como alternativa ao insucesso do tracionamento ortodôntico tradicional, sendo uma técnica mais conservadora que a exodontia, necessitando de conhecimento teórico e domínio técnico do profissional... Introduction: Apichotomy is a technique that has been used in teeth with severe root laceration in order to release the impacted tooth portion and thus allow its repositioning of the tooth in the dental arch. The aim of this paper is to present a clinical case report of orthosurgical orthodontic traction management associated with apichotomy in an included tooth with severe root laceration. Case report: The patient was referred to the Oral and Maxillofacial Service for extraction of the included element after ortho-surgical traction failure, which was verified by periapical radiographic examination, tooth included with orthodontic device and significant root laceration. Thus, under local anesthesia, apichotomy and orthodontic device installation were performed. The patient progressed well, and after 8 months the dental element was in occlusal position and in masticatory function. Final considerations: Thus, the apichotomy technique is effective as an alternative to the failure of traditional orthodontic traction, being a more conservative technique than extraction and requiring theoretical knowledge and technical mastery of the Professional... ABSTRACT

Introduction: This study analyzed dental consultation requests to the division of oral and maxillofacial surgery in a Brazilian tertiary hospital. Methods: A cross-sectional study with data collected retrospectively from inpatients' electronic medical records containing dental consultation requests made between January 2013 and December 2017. Results: 327 consultation requests were analyzed. Mean (SD) patient age was 38.71 (24.4) years; 164 (50.2%) were male and 267 (81.7%) were Caucasian. Regarding systemic conditions, 34 (10.4%) were classified as ASA I, 86 (26.3%) as ASA II, 182 (55.7%) as ASA III, and 25 (7.6%) as ASA IV. Dental consultations were mostly requested by the internal medicine team (n = 42, 12.8%). The most common reason for consultation was septic teeth (n = 131, 40.1%). Complementary tests were required in 188 (57.5%) cases. Surgical intervention was required in 82 (25.0%), with tooth extraction as the most prevalent procedure (20.2%). The most demanding service was inpatient care, with 276 (84.4%) requests. Cases were resolved in 249 (76.1%). Conclusions: The division of oral and maxillofacial surgery in our hospital deals with a great amount of consultations, contributing with surgical procedures to the adequacy of patients' oral health with a high effectiveness rate. Our data illustrate the contribution of dentists in a hospital setting, assisting the medical team in providing comprehensive care for inpatients. (AU)

ABSTRACT

Abstract Lesions in the floor of the mouth can be a challenging diagnosis due to the variety of pathological conditions that might be found in this area. Within a broad range of lesions, attention has to be addressed to those that require specific management, such as a dermoid cyst (DC) and a ranula. Especially in pediatric patients, in whom the failure of diagnosis can postpone the correct treatment and cause sequelae later in life. DC, a developmental anomaly, is managed primarily by surgical resection. On the other hand, ranula is a pseudocyst that may be treated by marsupialization. This article reports a large and painful lesion in the floor of the mouth in a pediatric patient. With a diagnostic hypothesis of ranula, two surgical interventions were performed, but there were recurrences of the lesion. Subsequently, the patient was referred to the Oral and Maxillofacial Surgery Unit for re-evaluation. Computed tomography showed a semi-transparent image suggesting a cystic formation. Another surgical procedure was performed where the lesion was completely removed. Anatomopathological analysis confirmed the diagnosis of DC. The five-year follow-up showed no signs of recurrence. This article indicates that although DC in the floor of the mouth is rare, it should be considered in the differential diagnosis of other diseases in this area. This precaution may be particularly important in the following circumstances: 1) Similar lesions that have different therapeutic approaches and, 2) To prevent future sequelae in pediatric patients.

ABSTRACT

Abstract Objectives This study aimed to evaluate the potential of adipose-derived stem cells (ASCs) combined with a modified α-tricalcium phosphate (α-TCP) or gelatin sponge (GS) scaffolds for bone healing in a rat model. Material and Methods Bone defects were surgically created in the femur of adult SHR rats and filled with the scaffolds, empty or combined with ASCs. The results were analyzed by histology and histomorphometry on days seven, 14, 30, and 60. Results Significantly increased bone repair was observed on days seven and 60 in animals treated with α-TCP/ASCs, and on day 14 in the group treated with GS/ASCs, when compared with the groups treated with the biomaterials alone. Intense fibroplasia was observed in the group treated with GS alone, on days 14 and 30. Conclusions Our results showed that the use of ASCs combined with α-TCP or GS scaffolds resulted in increased bone repair. The higher efficacy of the α-TCP scaffold suggests osteoconductive property that results in a biological support to the cells, whereas the GS scaffold functions just as a carrier. These results confirm the potential of ASCs in accelerating bone repair in in vivo experimental rat models. These results suggest a new alternative for treating bone defects.

ABSTRACT

Objetivo: avaliar a influência do número de imersões em ácido peracético na colonização bacteriana, na composição química, na rugosidade e na capacidade de recuperação após a deformação de tubos endotraqueais. Materiais e método: quatro tubos foram sub-metidos a sucessivas imersões em ácido peracético, constituindo quatro grupos: um controle (esterilizado pelo fabricante) e outros submetidos a uma, duas ou três imersões, de forma a simular o reprocessamento dos tubos. Os ensaios realizados foram: espectroscopia de infravermelho, rugosidade superficial, deformação da luz do tubo após compressão e colonização por Staphylococcus aureus. Resultados: o número de imersões (reprocessamento) não influenciou a colonização dos tubos por Staphylococcus aureus (p = 0,235), nem a composição química, nem a rugosidade (p = 0,621). Além disso, não houve diferença na capacidade do tubo em recuperar-se após deformação (p = 0,633). Conclusão: o reprocessamento por até três vezes não traz prejuízo às propriedades do material e não aumenta a colonização bacteriana na superfície dos tubos.

ABSTRACT The understanding of bone repair phenomena is a fundamental part of dentistry and maxillofacial surgery. Objective The present study aimed to evaluate the influence of buried magnetic field stimulation on bone repair in rat calvaria after reconstruction with autogenous bone grafts, synthetic powdered hydroxyapatite, or allogeneic cartilage grafts, with or without exposure to magnetic stimulation. Material and Methods Ninety male Wistar rats were divided into 18 groups of five animals each. Critical bone defects were created in the rats’ calvaria and immediately reconstructed with autogenous bone, powdered synthetic hydroxyapatite or allogeneic cartilage. Magnetic implants were also placed in half the animals. Rats were euthanized for analysis at 15, 30, and 60 postoperative days. Histomorphometric analyses of the quantity of bone repair were performed at all times. Results These analyses showed significant group by postoperative time interactions (p=0.008). Among the rats subjected to autogenous bone reconstruction, those exposed to magnetic stimulation had higher bone fill percentages than those without magnetic implants. Results also showed that the quality of bone repair remained higher in the former group as compared to the latter at 60 postoperative days. Conclusions After 60 postoperative days, bone repair was greater in the group treated with autogenous bone grafts and exposed to a magnetic field, and bone repair was most pronounced in animals treated with autogenous bone grafts, followed by those treated with powdered synthetic hydroxyapatite and allogeneic cartilage grafts.

ABSTRACT

Objectives: to compare the reproducibility of Puricelli's panorametry on conventional panoramic radiography and on CBCT reformatted panoramic images, acquired in MIP mode. This study also compared linear and angular measurements between both exams. Methods: were evaluated 54 pairs of conventional panoramic radiography and CBCT panoramic MIP reconstructions. panorametry was manually traced, in both films, by three examiners. The originally proposed linear and angular, dental and skeletal measurements where tabulated and compared between imaging methods. Inter-examiner reproducibility was tested by intraclass correlation coefficient, and the differences between measurements of both methods were analyzed by Student?s t test. Results: the inter-examiner correlation was high for both the conventional panoramic radiography and CBCT panoramic MIP reconstructions (0.86±0.07 and 0.88±0.07, respectively). Total magnification of pano-ramic radiograph, for the studied measurements, was of 4.3%, being vertically higher (12%) and to the horizontal inferior line (11.5%) comparing to the angular measurements of straight lines (6.7%) and to the horizontal superior lines (1.9%). The conventional panoramic radiography, in comparison to the CBCT, presented lower values for most angular measurements. Conclusions: Puricelli?s panorametry presents a high degree of reproducibility in both conventional panoramic radiography and CBCT reformatted panoramic images. Conventional panoramic radiography presents augmented linear and angular skeletal measurements and reduced dental angular measurements when compared to CBCT reformatted panoramic images. Conventional panoramic radiographs seem to present a consistent augmentation of the right side of the mandibular image.

ABSTRACT

The maxillary canines are amongst the most frequently impacted teeth, second only to the third molars. Several conservative orthodontic and surgical techniques are available to position the teeth properly in the dental arch, even in severe cases. However, when an extraction is necessary, it often leaves a critical alveolar defect of difficult management. The authors present the technique of Partial Maxillary Osteotomy, in which a dento-alveolar segment is moved mesially, hence closing the remaining space, allowing for the formation of healthy periodontium and resulting in an adequate functional and aesthetic outcome. A case report is presented with a 10 year follow-up, proving the technique's stability in the long term.

OBJECTIVES: The present study aimed at performing a histological evaluation of the response of temporal bone tissue to a change of direction of the force vector of the mandible in relation to the base of the skull. MATERIAL AND METHODS: Adult rabbits were assigned into four groups with two control and four experimental animals in each group. experimental animals underwent surgery, which resulted in a change of direction of the force vector on the right temporomandibular joint. Samples were collected after 15, 30, 60 and 90 days for histological analysis. RESULTS: In the two-way analysis of variance, the effect of group and time was statistically significant (p<0.001). Additionally, a statistically significant interaction between group and time was observed (p<0.001). Control animals showed normal growth and development of the temporal region. In the experimental group, the change in direction of the force vector of the mandible induced significant changes in the temporal bone, with a bone modeling process, which suggests growth of this cranial structure. CONCLUSIONS: The methodology used in this experiment allows us to conclude that the change in direction of the force vector of the mandible in relation to the skull base induces remodeling and modeling processes in the temporal bone. The resumption of normal oral functions after bone healing of the mandibular fracture appears to increase cell activation in the remodeling and modeling of the temporal bone structure. The observation of areas of temporal bone modeling shows the relevance of further investigation on the correlation between the joint structures and craniofacial growth and development.

INTRODUCTION: In the present study, the stability and biocompatibility of a 30 percent polymethylmethacrylate (PMMA) filling material implanted in the masseter muscle of rats were investigated according to the cytologic characteristics presented in the graft versus host reaction. METHODS: The study included 20 rats, which were divided into 4 groups: groups I, II, III, and IV corresponded to animals evaluated 7, 14, 45, and 60 days after surgery, respectively. The implant was placed in the right masseter muscle at the level of the mandibular angle. RESULTS: After 7 days, lymphoplasmacytic inflammatory infiltrates, a fibrous capsule, a large number of neutrophils, macrophages, and exudate were observed. The second group (14 days) showed granulation tissue composed of a lymphoplasmacytic inflammatory infiltrate, newly formed vessels, and a fibrous capsule. However, the second group also exhibited regeneration of the muscle fibers, and a decreased number of neutrophils and exudate. After 45 and 60 days, the inflammatory infiltrate decreased in intensity compared to the first 2 groups. CONCLUSIONS: The inflammatory reaction caused by PMMA is transient and does not compromise the function and the shape of the masseter muscle tissue, suggesting that PMMA is biocompatible.


INTRODUÇÃO: Este trabalho busca avaliar, em ratos, a estabilidade e a biocompatibilidade de um material de preenchimento à base de polimetilmetacrilato (PMMA) a 30 por cento implantado no músculo masseter, por meio do padrão, e a organização reacional no tecido receptor. MÉTODO: Foram utilizados 20 ratos, divididos em quatro grupos: grupo I, que correspondeu ao período de 7 dias de pós-operatório; grupo II, de 14 dias; grupo III, de 45 dias; e grupo IV, de 60 dias. O implante foi realizado no músculo masseter direito, na região do ângulo da mandíbula. RESULTADOS: No período de 7 dias, observou-se presença de infiltrado inflamatório linfoplasmocitário, com formação de cápsula fibrosa e presença de grande número de neutrófilos, macrófagos e formação de exsudatos. Em 14 dias, observou-se a formação de um tecido de granulação composto por infiltrado inflamatório linfoplasmocitário, vasos de neoformação e cápsula fibrosa. Porém, nesse tempo experimental, nota-se a regeneração das fibras musculares e a diminuição do número de neutrófilos e exsudatos. Após 45 dias e 60 dias, observou-se, no tecido muscular, diminuição da intensidade do infiltrado inflamatório, comparativamente aos tempos experimentais anteriores. CONCLUSÕES: A reação inflamatória provocada pelo PMMA é transitória e não compromete as funções e o contorno desse tecido muscular, o que sugere que o PMMA é biocompatível.

ABSTRACT

Objective: This in vivo experimental study evaluated the effect of methotrexate on the oral mucosa of mice. Methodology: A total of 28 adult BALB/c mice were randomly assigned into a control and an experimental group. Mice in the experimental group were immuno-suppressed with 2.5 mg/kg of methotrexate for three consecutive days. One mouse from each group was sacrificed each day for 10 days. Samples from ventral mucosal tongue surface were collected and prepared for histomorphometric analysis, by staining with hema-toxylin/eosin. Three microscopic s per section were analyzed, in order to evaluate the mean epithelial cells layer thickness and counting of blood vessels. To eva-luate the existence of inflammatory infiltration, values of 0 and 1 were used for absence or presence, respec-tively. Results: The results showed that the treatment did not induce clinical signs of mucositis in the ventral mucosal tongue region. The histological examination showed that the mean number of blood vessels was si-milar in control and experimental samples (p < 0.001), and that there was absence of inflammatory infiltration. However, the thickness of the epithelial cell layer of experimental samples was significantly lower than in control samples. Conclusion: This study showed that, in the mouse model, methotrexate-induced mucositis histological result in manifestations in the ventral tongue region.

The lateral cephalometric radiographs show a high degree of visual resolution for the pharyngeal airway space, which is delimited by soft and hard tissues. Oro-maxillofacial surgical treatment may be indicated for the patient with skeletal class II malocclusion, where whether or not mandibular advance is associated with interventions of the maxilla. Objective: The objective of this study was to determine the stability of the dimensional alterations of the total pharyngeal airway space (TPAS), distinguishing the airway space of the nasopharynx (NP), oropharynx (OP) and hypopharynx (HP), in cm2, after maxillomandibular advance. Methods: Cephalometric radiographs were taken in the preoperative (PRE), early postoperative (EPOST) and late postoperative (LPOST) periods of patients who submitted to combining surgeries. Based on proposals of cephalometric studies for this area, a statistical analysis was accomplished to determine the presence or absence of significant differences between PRE and EPOST, between EPOST and LPOST, and between PRE and LPOST. Results: The-re was an increase in values of the OP area and of TPAS in EPOST and decrease in LPOST. However, for these airway spaces, area values in LPOST differed from those in PRE, which were always much lower. Conclusions: These results may contribute to the treatment of obs-tructive sleep apnea and hypopnea, even if relapse occurs, where the pharyngeal airway space is still greater in LPOST than in PRE.

Objetivo: O presente trabalho propõe o estudo histológico descritivo da aplicação de um campo magnético sepultado, estático e permanente em área de enxerto ósseo alógeno liofilizado e dentoalveolar associado a defeito cirúrgico em mandíbulas de ratos. Metodologia: Estudo experimental in vivo, randomizado. A amostra foi composta por 21 ratos, da espécie Rattus novergicus albinus, linhagem Wistar, machos, divididos em três grupos correspondendo aos tempos experimentais de 7, 21 e 45 dias. Foram realizadas ostectomias as-sociadas a odontossecções na face lateral externa do corpo mandibular e no ramo ascendente, atingindo transversalmente a raiz do incisivo inferior e os ápices dos molares. Esse defeito cirúrgico permitiu a adaptação de dispositivos metálicos magnetizados (grupo teste) e não magnetizados (grupo controle) associados às regiões apicais dos molares e ao coto distal do incisivo inferior, local onde se realizou enxertia óssea alógena liofilizada. A intensidade média do campo magnético sepultado e permanente foi de 250 gauss. Resultados: Observou-se no grupo teste e controle, nos diferentes tempos experimentais, a gradativa integração do enxerto ósseo alógeno liofilizado, a manutenção da vitali-dade pulpar dos molares e coto proximal do incisivo inferior, além da contínua erupção do incisivo inferior do rato. Nos grupos teste, principalmente aos 45 dias, constatou-se uma diferenciada e exuberante neoformação óssea centrípeta em direção aos dispositivos metálicos imantados. Conclusão: o campo magnético sepultado in vivo foi capaz de favorecer o processo de cicatrização óssea no defeito cirúrgico criado.

ABSTRACT

Objetivo: Este trabalho busca avaliar, por meio da observação macroscópica, a adesão e, por resultado histológico, as reações teciduais e a biocompatibilidade dos materiais após a utilização de adesivos empregados em prótese bucomaxilofacial em epiderme de aves. Métodos: Foram utilizadas oito aves da raça Gallus Galllus Domesticus (GGD), divididas em quatro grupos com duas aves cada. O Grupo I correspondeu ao adesivo Factor II B-200-ES - Daro Adhesive Extra Strength®; Grupo II, ao adesivo para cílios postiços Wimp-rs®; Grupo III, à cola bastão Pritt®; Grupo IV, à adesivo de cílios postiços de fabricação caseira. Um disco de silicone contendo o referido adesivo foi fixado à epiderme cervicodorsal dos espécimes GGD. Para controle da inflamação quanto à deplumagem foram utilizadas duas aves da linhagem pescoço pelado (PP). Os resultados da análise microscópica e macroscópica do Grupo GGD e do Grupo PP foram obtidos em três e sete dias após a colocação dos adesivos. Resultados: Todos os adesivos se mostraram inócuos, sem qualquer reação inflamatória associada. O adesivo Factor II, B-200 Extra-Strenght® e o adesivo para cílios postiços de fabricação caseira são eficazes quanto à retenção. Os adesivos Factor II, B-200 Extra-Strenght®, adesivo para cílios postiços Wimp-rs®, cola em bastão Pritt® e adesivo para cílios postiços de fabricação caseira são inócuos e biocompatíveis. Conclusões: Sugerem-se novos estudos, que avaliem especificamente a retenção dos adesivos e as alterações de temperatura e umidade ambientes.

ABSTRACT

Purpose: Magnetic resonance imaging (MRI) is useful for interpreting temporomandibular joint (TMJ) disorders including disc displacement. This study aimed to evaluate the relationship between TMJ sounds and disc displacement with and without reduction by clinical examination and MRI. Methods: A sample of 58 patients was clinically examined when after being referred to MRI due to signs and symptoms of unilateral or bilateral TMJ disc displacement. The diagnosis obtained using MRI was compared with the clinical findings for the presence or absence of articular sounds. Results: Most patients (n=39; 67%) had joint sounds manifested as clicking; the other 19 patients (33%) did not present sounds. Among the 39 patients with sounds, bilateral clicking was observed in 29 (74%), and unilateral clicking in 10 (26%) patients. Unilateral clicking was equally observed on the right and left sides, each occurring in 5 five patients (13%). Conclusion: The results suggest that articular sounds do not occur exclusively in TMJ with disc displacement with reduction. The presence or absence of joint sounds will not suffice as the only aspect criterion to be considered in the differential diagnosis of disc displacement with and without reduction.


Objetivo: A ressonância magnética nuclear (RMN) é de grande utilidade para o diagnóstico das desordens temporomandibulares, inclusive o deslocamento de disco é muito comum. O objetivo deste estudo foi avaliar a relação entre ruído articular e deslocamento de disco com e sem redução por meio de avaliação clinica e ressonância magnética nuclear. Metodologia: O estudo examinou 58 pacientes clinicamente, que foram submetidos à RMN quando apresentavam de sinais e sintomas de deslocamento de disco uni ou bilateral. O diagnóstico obtido pela RMN foi comparado com os achados clínicos relativos à presença ou não de ruído articular. Resultados: Dos 58 indivíduos estudados, 39 (67%) apresentaram ruído articular do tipo click, os restantes 19 (33%) não apresentaram ruídos. Dos 39 com ruído, a ocorrência bilateral foi de 29 (74%) e unilateral em 10 (26%) casos, sendo 5 (13%) do lado direito e 5 (13%) do lado esquerdo. Conclusão: Os resultados demonstraram que os ruídos articulares não estão restritos a articulações com deslocamento de disco com redução. A presença ou ausência de ruído articular não devem ser os únicos aspectos considerados para o diagnóstico diferencial entre deslocamento de disco com e sem redução.

ABSTRACT

Purpose: To present a case of total bilateral jaw necrosis in a patient with primary breast cancer and bone metastases treated with zeledronic acid and discuss the adverse effects of biphosphonates and the best moment to perform dental procedures. Case description: A female patient, 62 years old, with primary breast adenocarcinoma underwent chemotherapy and zoledronic acid because of bone metastases on the right hip. After 7 months, she was submitted to debridement of exposed bone on the anterior mandible, which resulted in skin fistulae and severe pain. Computerized tomography showed bone sequestration hypodensity at the right and left mandibular body and mentum with excessive uptake of the labeled drug as seen by bone scintigraphy. The patient received amoxicillin, gluconate and chlorhexidine mouthrinse and hydrogen peroxide; partial closure of skin fistulae was achieved. After 4 months, the patient returned with submandibular tissue necrosis, intra- and extra-orally exposed mandible with active discharge. After 5 days of palliative treatment, the patient died from sepsis. Conclusion: Local infection/inflammation should be treated before the use of biphosphonates, and topic and systemic pharmacologic treatment should be combined with a strict follow-up. For selected cases, the osteonecrosis treatment may include conservative debridement, pain control, topic and systemic antimicrobial control, and, in extremely severe cases, radical surgery.


Objetivo: Apresentar um caso de necrose total de mandíbula, bilateral, em uma paciente com neoplasia primária de mama e metástases ósseas tratadas com ácido zoledrônico endovenoso, e discutir os efeitos adversos de bisfosfonatos e o momento adequado para realização de procedimentos odontológicos. Descrição do caso: Paciente do sexo feminino, 62 anos, com adenocarcinoma de mama primário foi submetida a quimioterapia e uso de ácido zoledrônico devido a metástases ósseas no quadril direito. Após 7 meses, realizou-se debridamento de osso exposto na região anterior da mandíbula, resultando em fístulas e dor severa. Os exames de tomografia computadorizada e de cintilografia óssea revelaram hipodensidade do sequestro ósseo no corpo mandibular e no mento com presença excessiva da droga nestes sítios. Após antibioticoterapia e uso de enxaguatórios bucais houve fechamento parcial das fístulas. Após 4 meses, a paciente retornou com necrose submandibular e osso mandibular exposto intra e extraoralmente com drenagem. Após 5 dias de tratamento paliativo a paciente morreu de sepsis. Conclusão: Focos de infecção e/ou processos inflamatórios locais devem ser tratados previamente ao uso de bisfosfonatos. Indica-se o tratamento farmacológico tópico e sistêmico, e a proservação do caso. Terapias para osteonecrose, em determinados casos, envolvem debridamento conservador, controle da dor, manejo antimicrobiano tópico e sistêmico e, em casos extremos, cirurgia radical.

ABSTRACT

O fosfato de cálcio previamente moldado pode ser utilizado pode ser utilizado para reconstruções em regiões bicomaxilofacias. A esterilização prévia desse material é um importante passo na prevenção de porcessos inflamatórios e infecciosos. No presente trabalho foram utilizados quatro blocos de cimento de fosfato de cálcio (Ca (PO) ), divididos em quatro grupos, cada qual contendo uma peça. Os processos de esterilização para os três grupos experimentais foram: autoclave ou calor úmido (Grupos 1a e 1b), peróxido de hidrogênio (Grupos 2a e 2b) e óxido de etileno (Grupos 3a e 3b). O grupo de controle (Ca e Cb) não foi submetido à esterilização. Inicialmente, cada bloco foi semeado em tudo contendo caldo tripticase de soja (TSB) (Grupos a). Os meios líquidos resultantes foram semeados em placas de Petri contendo ágar sangue (Grupos b). Os meios de cultura sofreram processo de incubação em aerobiose. Os resultados foram determinados pela turvação ou não dos meios líquidos e pela presença ou não de colônias bacterianas nos meios sólidos. Ao contrário do grupo de controle, não houve crescimento bacteriano em nenhum dos grupos experimentais. O resultado do exame bacterioscópico ou método de Gram do grupo de controle mostrou numerosos Staphylococcus sp e Streptococcus sp. Conclui-se que os métodos de esterilização testados calor úmido ou autoclave, peróxido de hidrogênio e óxido de etileno são eficazes para aeróbios. Recomenda-se a utilização do processo de esterilização de blocos de fosfato de cálcio para implantação in vivo. Sugerem-se novos estudos para avaliação das propreidades mecânicas deste material após o processo de esterilização.

ABSTRACT

A presente pesquisa teve por objetivo realizar uma avaliação comparativa entre os resultados das medidas obtidas segundo dois traçados (Altonen, Haavikko e Mattila × Puricelli), efetuados sobre radiografias panorâmicas em dois momentos de formação radicular, verificando-se a presença ou ausência de compatibilidade entre os mesmos quanto ao grau de inclinação dos terceiros molares inferiores em relação aos segundos molares. Desta forma, as seguintes medidas foram comparadas: ângulo Beta de Altonen, Haavikko e Mattila correspondente ao gama de Puricelli, o qual indicou o relacionamento dos terceiros molares inferiores em relação aos segundos molares. Baseando-se nos resultados obtidos, apresentados e discutidos no presente trabalho e de acordo com a metodologia estudada, pode-se concluir que os traçados diferem entre si. Entretanto, apresentam uma correlação positiva.


The aim of the present investigation was to compare the measurements obtained based on two tracings (Altonen, Haavikko and Mattila versus Puricelli) conducted on panoramic radiographs at two moments of root formation, as to their compatibility with the degree of inclination the lower third molars in relation to the second molars. Thus, the following measurements were compared: the beta angle of Altonen, Haavikko and Mattila with the corresponding gamma angle of Puricelli, parameters that indicate the relationship of the lower third molars to the second molars. Based on the results presented and discussed in the present study, and based on the method employed, it is concluded that tracings differ but also show a positive correlation.

ABSTRACT

O tratamento ortodôntico envolve a abordagem de alterações da oclusão desde a dentadura decídua até a permanente. No período de transição da dentadura mista para a permanente poderão ocorrer problemas de impactações dentárias. Neste trabalho, os autores relatam um tratamento multidisciplinar ortodôntico-cirúrgico realizado em uma paciente de nove anos de idade. A paciente apresentava um incisivo central superior direito retido e impactado no palato. A opção pelo tratamento combinado ortodôntico e cirúrgico se mostra eficiente, principalmente quando bem diagnosticada e executada por meio de técnica adequada.
